Cut the bridge, go down, light the Old King's Antechamber bonfire, run past the Sandworm and light the Demon Ruins bonfire. Defeat the Old Demon King. To save Greirat from Irithyll, you have to save Siegward from the well outside the Church before the Cathedral of the Deep by buying his armor from Patches and giving it to him. Or maybe just what Patches said "Greirat did him a favour" in Lothric dungeon and he's grateful for it. Patches is an asshole.
